OFCCP Updates Voluntary Self-Identification Form

Article contributed by Mark Adams, EANE

The Office of Federal Contract Compliance Programs (OFCCP) has announced that it has revised its Voluntary Self-Identification Disability Form (CC-305).  Federal contractors and subcontractors who have affirmative action requirements are required to undergo efforts to collect information from both applicants and new hires concerning their disability status and they additionally have responsibilities to collect that information from the workforce every five (5) years.

Unlike similar responsibilities to collect sex, minority and veteran status information on applicants and new hires (whereby an employer is left to develop their own form for that purpose), the OFCCP has a model form for collecting information on disability status and affirmative action employers may not:

  • Alter the content (i.e., text or wording) of the form;
  • Alter the order of the content on the form; and
  • Alter the form or make changes that diminish the general accessibility of the form

Notwithstanding those conditions, the OFCCP is giving affirmative action employers time to implement the new form – until July 25, 2023.

The form can be found here:  Voluntary Self-Identification of Disability Form (CC-305)
